Kindergarten Registration Forms
2012-2013 School Year

The following forms are in Adobe Acrobat Format.
You will need to print the forms, fill them in and bring them with you to Kindergarten Registration, Feb 25th through March 1st, 8:30 - 4:00 each day.

Forms will also be available on the day of registration.

Other documentation needed includes:
Birth Certificate
Immunization Record
Social Security Card
Custody Papers (if Applicable)

Children do not need to be present on the day of registration.
